Evergreen Health

Evergreen Health fosters healthy communities by providing medical, supportive and behavioral services to individuals and families in Western New York — especially those who are living with chronic illness or are underserved by the healthcare system. We’re a non-profit community health center serving some of our region’s most vulnerable patients, and we never turn anyone away due to an inability to pay.
